Renewal of Certificate

  1. Filled Application for Renewal.
  2. Original ink-signed copy of the dealership or distributor agreement if the earlier submitted dealership agreement has expired.

Modification of Certificate

  1. Filled Application for Modification.
  2. Original ink-signed copy of the dealership or distributor agreement for additional equipment
  3. Brochure of the additional equipment
